Your role and responsibilities
- Be a senior member of the front line Home First team, providing guidance for Reablement Support Workers (RSWs) and supporting the Team Leader in their leadership role.
- Meet the immediate needs of new customers to the service, whilst supporting independence at all times.
- Contribute to customer ongoing reviews to promote customer’s views and share information on Home First understanding of areas of need.
- Support customers to achieve reablement goals aimed at promoting independence and choice.
- Support customers with care needs on discharge from hospital to get back to independent lives in their own homes
- Inform and promote future options for meeting customer’s long-term needs where these remain following Home First interventions/enablement.
- Provide job shadow support and mentoring for new staff, competing observations where required Collaborating with Home First Assessment staff to carry out assessment and goal setting for Home First customers.
What you will need to do in your role
- Establish and maintain effective and positive relationships with customers and family carers, to promote the health, well-being, and independence to the customer.
- Engage with customers, family carers and others to promote, inform and explore alternative ways to meet and maintain their long-term needs.
- Inform and promote a range of services to help people live independently and safely at home.
- Monitor customers progress against Home First care plans and complete records as required to inform the ongoing review process.
- Maintain and contribute to accurate and comprehensive records to support the care plan and inform the review process.
- Be the first point of call for initial RSW queries regarding customers` care and daily issues and to be competent to recognise when something needs to be referred up to a Team Leader for action.

The team
Home First provides a short term assessment and reablement care and support service to people living in their own homes. This may be following a stay in hospital, an illness or crisis at home, or help to avoid an admission into hospital. You will be working closely with our colleagues from the Integrated Neighbourhood Teams (INTs) which include Social Work Teams, Community Health, Independent Providers, GPs the local Councils, voluntary services as well as many others. You will also work closely with Safeguarding colleagues.
